Add Advice by Licensed Surveyor

Advice by Licensed Surveyor (Form 13) can be provided to the Responsible Authority in SPEAR.

To add the “Advice by Licensed Surveyor (Form 13)”, or indicate that the advice by the Licensed Surveyor is not required; the following steps need to be completed:

Steps

1. Select the other action "Add Advice by Licensed Surveyor (Form 13)" from the Details Screen and click 'go'.
2. If advice by Licensed Surveyor is not required, select the radio button option: "Advice by Licensed Surveyor is not required for this Application"
3. If advice by Licensed Surveyor is to be provided, select the radio button option "This is the written advice by a licensed surveyor required by section 20A of the Subdivision Act 1988…"
   The screen will then expand to show additional options. Tick the relevant items and, depending on the items selected, complete the remainder of the form by choosing the required radio buttons or calendar date.
4. Click ’add now’.
5. To complete the process, the Advice by Licensed Surveyor (Form 13) must be authenticated (signed) using a Digital Certificate.
